I understand I am accepting care from Ashley Goodwin, a Fertility Awareness and Sexual/Reproductive Health Educator.  I am aware that she is well trained in the fields of reproductive and sexual health and may hold other degrees or certifications.

I also permit my charts, chart notes and intake to be used anonymously by The Well and The Hippie Martha for research and teaching purposes in the future.

I understand that Ashley Goodwin is not a doctor and all advice and suggestions given are not intended to diagnose or treat. I hold full and complete responsibility for my health care, my contraception and my fertility choices.  I will not hold Ashley Goodwin responsible for any undesired outcomes.

Enrolling in any class that Ashley Goodwin teaches, I agree (to the best of my ability) to view or  attend all classes, staying for the entirety (and with camera on if online), to pay the requested tuition in full before the end of the class series or if I need financial assistance to work directly with Ashley prior to class.
